How to Apply
The application form must be filled in, and returned to us along with all relevant documents:
- Personal Licence Application Form
- Relevant Licensing Qualification
- A Convictions and Cautions Declaration Form
- Basic DBS Certificate
- Proof of right to work in the UK
- 2 countersigned passport photographs
The form should be fully completed and accompanied by the specified fee.
We will need to see the original version of your Disclosure and Barring Certificate.
