People leaving hospital
If you are at risk of homelessness because you are leaving hospital and have no home, or your current accommodation has become unsuitable, please read the information below.
If you are in hospital and you have no home or your current accommodation has become unsuitable, let a member of the ward staff know as soon as possible.
Some larger hospitals (such Royal Devon and Exeter, Derriford and Torbay Hospital) may have a Discharge Coordinator. They can help you consider your options and work with in partnership with the Housing Team to identify the most suitable outcomes for your needs.
If they do not have a Discharge Coordinator then we can arrange for a member of the team to make contact with you to go through your housing options and support you when you are discharged. Ask a member of the Ward Staff to get in touch with the Housing Advice team.
If we can, we will work with you to help you stay in your current home. This could be anything from negotiating with your current landlord to helping install Disabled Adaptations to your property to make it safe and comfortable for you to remain there.
The Housing Options Team will work with you to develop a Personal Housing Plan to help you resolve your housing difficulty and provide you with written advice tailored to your needs. They will work closely with you, the hospital and any other relevant agencies such Occupational Therapists and Social Services to develop the right solutions for you.
